Very Easy Steps to Complete:

  1. Print Petition (8.5x11)

    1. Print front/back one page.

  2. Top Right of form checkbox “volunteer” and place your county.

  3. Sign the form.

    1. You can get more signatures 10 per page or just you sign the page.

  4. Get notarized.

  5. Mail to me:

Step-by-Step Directions

Want to help collect signatures?

  • Must be registered to vote in Arizona

  • Must be a resident in Arizona

  • Click the PDF button, download PDF, keep the information placed in the PDF on the form, print off form.

  • Collecting signatures MUST be in blue or black and printed clearly and in the box.

  • One county per petition form only.

  • Signatures must be witnessed by the circulator of the signatures.

  • Each circulating form must be notarized by the circulator with a registered notary.

  • Lastly: Mail to me original signed and notarized form:
